The real problem

More facts alone will not fix your performance under pressure.

You can know the protocol and still feel the weight of uncertainty when the scene is messy, the diagnosis is unclear, the room is loud, the team is waiting, and the next decision belongs to you.

You may be new and finding your footing. You may be experienced and frustrated that you have plateaued. You may be training others and trying to explain judgment, communication, and leadership skills that usually only get learned the hard way.

The common thread is not a lack of caring. EMS asks clinicians to lead under uncertainty, but gives very few of us a structured way to practice that before the call demands it.

You replay calls after shift.

Not because you are careless, but because you care enough to wonder what you could have done better. Replaying without a framework turns into spiraling instead of learning.

You feel yourself plateauing.

More CE gives you more information, but not always a better decision-making loop, better communication, or better command presence.

You are expected to lead in real time.

Newer medic, experienced paramedic, rural clinician, field trainer, or transport clinician — people look to you when patient care needs direction.

The shift

Confidence is trained.
Mastery is practiced.

The old way is to collect more facts, memorize more protocols, and hope confidence comes with time.

The Fellowship is built on a different idea: confidence comes from coached reps, case-based thinking, emotional regulation, team communication, peer support, and mentorship feedback loops.

The goal is not to become a clinician who never feels uncertainty. That person does not exist. The goal is to become someone who can stay clear, communicate well, and keep moving when uncertainty is part of the job.

“Paramedic school taught me medicine. This course taught me how to be a well-rounded, cool, calm, and collected provider.”

— Spencer
The 12-month arc

A clear structure for getting better at the craft.

It starts with clarity, then builds your decision-making frameworks, then compounds into better scene leadership and professional identity.

00
First 30 days

Know where you stand

Complete the Paramedic Efficacy Assessment, receive your SimKit, join the community, and meet 1:1 with Ross or Will to review your results and build your action plan. You leave the first month with a personal development plan specific to you and your goals: what to work on, what to practice first, and how to start becoming the clinician you want to be.

01
Months 1–3

Build your decision-making loop

Work through the Foundations curriculum, attend coaching calls, shadowbox cases, use the workbook, and build a habit of practicing high-acuity, low-occurrence procedures. The aim: less spiraling, better reflection, clearer communication, and more deliberate decisions on your next shift.

02
Months 4–6

Apply it on shift

You meet 1:1 again to review progress and revise your personal development plan, then translate the frameworks into real field behavior: leading clearly on scene, delegating earlier, communicating your plan, noticing when you get task-focused, and using after-action review to turn calls into useful learning reps instead of vague anxiety.

03
Months 7–9

Deepen the craft and coach the process

Another 1:1 review, then deeper pattern recognition, more advanced case discussion, stronger team communication, and learning how to explain your thinking to others.

04
Months 10–12

Become the clinician people look to

A final 1:1 review and a plan for the year's last quarter. You lead scenes with more presence, sit easier in uncertainty, and help your team get on the same page faster. By year's end, others will notice the change: calm, competent, humble, and worth looking to when the call is complicated.

“I started finding my voice, starting to delegate, and communication with other agencies is improving. Basically I'm less scared when I get out of the ambulance now.”

— Lindsey
Fit check

This is for EMS clinicians who are serious about growth.

It is not limited to one career stage. It is for clinicians who lead patient care in the field and want to get better at the parts of EMS that matter most when things get complicated.

This is for you if…

You lead patient care in the field under time pressure and uncertainty.
You are an EMS clinician at any career stage who is hungry to get better, not just maintain a credential.
You work rural or remote and may have to lead at the top of your local scope with limited backup.
You work urban and want efficient practices that lead to better patient care in the short time before the hospital.
You are a field trainer, preceptor, FTO, or experienced medic who wants clearer principles for coaching newer clinicians.
You are an EMT or paramedic student preparing for internship, transition, or early field leadership.
You work in critical care transport and want to think and lead better in high-pressure situations.
You want coaching, reps, feedback, a community of motivated like minded clinicians, and a structured path toward mastery.
You want to become the calm, competent person others trust when the call is complicated.

The value comes from doing the work, not from passive consumption.

This is not for you if…

×You want a quick certification or passive video course.
×You think more knowledge alone will solve your uncertainty, anxiety, or leadership problem.
×You do not enjoy learning with others or learning from others.
×You believe you have nothing to learn from clinicians with less experience than you.
×You expect someone else to make you better without doing the work yourself.
×You are in love with the outcome but unwilling to fall in love with the process.
×You are unwilling to participate in coaching, reflection, case practice, or honest feedback.
×You think confidence means never being uncertain.

Included with membership

A SimKit so rare skills do not stay rare in practice.

Every new annual member receives a personalized procedure kit for practicing cricothyroidotomy, IOs, needle and finger thoracostomy, and related procedural skills.

The point is not owning equipment. It is building a practice rhythm for skills you may not perform often, but still need to perform under pressure.

It is paired with online learning modules and a procedural competency calendar for each procedure, so you can keep these skills alive between real-world exposures.
